Proposed membership policies

Before you enroll

Memberships have a 12-month commitment. A membership is not health insurance. Before enrollment, we will provide a written agreement identifying the plan, included services, charges, billing schedule, renewal terms, and cancellation process. No payment is requested through this website. These draft terms do not create an enrollment or authorize a charge.

Requesting cancellation

Send written requests to info@goodrootsmedicine.com with your name and preferred callback details. Please do not include medical information. We will confirm receipt and the proposed end date in writing. A request alone does not cancel the agreed 12-month term. Early termination requests, including relocation, medical circumstances, or financial hardship, will be reviewed individually under the signed agreement and applicable law.

After the initial term

The proposed policy is 30 days’ written notice before the requested end date after the initial 12-month commitment. Renewal and any recurring billing require clear disclosure in the final agreement. We will explain any remaining balance before processing a cancellation.

Refunds and unused services

Our proposed policy is to refund unused prepaid membership time after an approved termination date on a prorated basis, less amounts properly due under the signed agreement. Completed visits and treatments are not refundable solely because a membership ends. Unused benefits do not automatically create a cash refund. Any treatment-bank balances, credits, or package-specific provisions will be stated separately before purchase; this draft creates no forfeiture or cancellation fee.

If the practice cancels or opening changes

If we cannot provide a prepaid membership or cancel it, the proposed policy is to return payment for the unprovided portion. We will not charge for a pre-opening period in which membership services are unavailable. Refunds, once approved, are proposed to be returned to the original payment method within 30 days, subject to payment-provider processing.

Care transitions and appointment cancellations

Membership cancellation does not replace an orderly care transition or your right to request records. Any appointment cancellation, no-show, or late-cancellation fee will be disclosed separately before booking; none is established by this draft.

Replacing this draft

The signed membership agreement and applicable law will govern enrollment. Final terms will be available before any payment or commitment. Changes will not be applied retroactively to an existing agreement without the notice or consent required by that agreement and law.
